Is there a way to assign a condition rating “1-5” to my assets, update the rating from 6 monthly visual assessments and track the changes in condition rating over time?

Out of curiosity, Could you expand on aim of such score system in more detail?
Reporting side asset health lists good overview on asset condition based on WO generated over period.
But I believe you want something specific. Procedures support scoring system which you could trigger corrective actions. Not sure if scoring goes to reporting side, but assume Maintainx team could do something. You could potentially utilise condition meters on top for each asset. So as example engineer does assessment using procedure with scoring then uses asset meter field to enter final score. All scores you entered can view in meter graph plus set additional triggers. Maybe not exactly what you need but it might be usable workaround.
